General Terms and Condition

  • Patients choosing Molonglo dental may be asked to read and sign our consent form prior to treatments.
  • Please be aware that we require a minimum of 12 hours’ notice should you need to cancel or reschedule an appointment. Failure to provide 12 hours’ notice may incur a fee and/or require a deposit to secure another booking at Molonglo Dental.
  • If you are a new patient, please arrive 10 minutes before your appointment time to complete a new patient form. Please bring your Medicare card and any concession or health care cards.
  • If you have any cold or flu-like symptoms, have travelled overseas or interstate in the last 14 days or have been in contact with a suspected case of COVID-19, please inform our staff before attending your appointment.
  • The payment plan facility provided by the clinic cannot be used in conjunction with Smiles membership.
  • All patient personal and medical record including, a copy of notes, radiographs and photographs will be handled and made available in accordance with the Privacy Act 1988.
  • Surgery reserve the right to ask for any competitive treatment price quote and reserve right to refuse to match or beat any competitive offer price.
  • Molonglo dental reserve the right to refuse any cosmetic treatment to patients, if it is suspected of any adverse oral health outcomes.
  • Health fund limits and rules apply for private health insurance customers. Please confirm eligibility with health funds provider prior to appointment to claim rebates with the Molonglo dental surgery.
  • If Patient has used their yearly limit from their health fund, please contact Molonglo dental for a price estimate. This estimate is within the range and does not guarantee a fixed price quote.
  • A comprehensive consultation appointment will be required from treating dentist to assess a specific dental condition for obtaining an exact quote.
  • The exact quote on treatment plan will only be valid for 30 days.
